چکیده
Objective: The objective of this study was to identify high risk factors for survival in locally advanced rectal cancer patients with ypT0-2N0 after neoadjuvant chemoradiation. Methods: In this study, Surveillance, Epidemiology, and End Results Program (SEER)-registered rectal cancer patients, and patients from Fudan University Shanghai Cancer Center (FUSCC) after preoperative chemoradiation were combined to identify poor prognostic patients with ypT0-2N0. Results: 2375 patients from SEER data and 106 patients from FUSCC were analyzed. The SEER data showed that less than 12 lymph nodes and mucinous/signet ring cancer were the two risk factors. The 5-year cancer specific survival (CSS) in patients with none, one or two risk factors was 91.5%, 88.5% and 70.3%, respectively (P < 0.001). Our cancer center data showed that less than 12 lymph nodes and mucinous/signet ring cancer were also the two risk factors. The 3-year disease free survival (DFS) in patients with none, one or two risk factors was 96.2%, 85% and 37.5%, respectively (P = 0.013). Conclusions: Two risk factors were identified in this study that correlated independently with a worse survival in patients with ypT0-2N0. The current results indicated that adjuvant chemotherapy may be not spared in these patients who have 1 or 2 risk factors.

Implications for selecting local excision in locally advanced rectal cancer after preoperative chemoradiation
Local excision may offer the possibility of organ preservation for the management of locally advanced rectal cancer after neoadjuvant chemoradiotherapy (CRT). However, the oncological outcomes of this strategy have been largely associated with the risk of nodal metastases. In this study, Surveillance, Epidemiology, and End Results Program (SEER)-registered rectal cancer patients, and patients f...
متن کاملYpT1-2N0 rectal cancer after neoadjuvant chemoradiation has lower survival compared with pT1-2N0 rectal cancer.
Pathologic T1-2N0 rectal cancer shows an excellent prognosis without preoperative or postoperative chemoradiation. However, oncologic outcome of ypT1-2N0 remains unclear and undetermined. Thus, the aim of this study was to compare the survival of ypT1-2 and pT1-2 rectal cancer patients after radical resection and identify risk factors of ypT1-2 rectal cancer in Surveillance, Epidemiology, and E...
متن کاملHelical Tomotherapy Combined with Capecitabine in the Preoperative Treatment of Locally Advanced Rectal Cancer
The aim of this study was to evaluate the efficacy of helical tomotherapy plus capecitabine as a preoperative chemoradiotherapy (CRT) in patients with locally advanced rectal cancer (LARC). Thirty-six LARC patients receiving preoperative CRT were analyzed. Radiotherapy (RT) consisted of 45 Gy to the regional lymph nodes and simultaneous-integrated boost (SIB) 50.4 Gy to the tumor, 5 days/week f...
متن کاملCirculating Cell-Free DNA in Plasma of Locally Advanced Rectal Cancer Patients Undergoing Preoperative Chemoradiation: A Potential Diagnostic Tool for Therapy Monitoring
Circulating cell-free DNA opens up an interesting field for therapy monitoring, in particular during multimodal therapy protocols. The objective of this proof of principle study was to evaluate whether the amount of circulating plasma DNA has the potential to serve as a marker for therapy monitoring during the treatment course of locally advanced rectal cancer patients. We especially focused on...
متن کاملValidation of a rectal cancer outcome prediction model with a cohort of Chinese patients
The risk of local recurrence (LR), distant metastases (DM) and overall survival (OS) of locally advanced rectal cancer after preoperative chemoradiation can be estimated by prediction models and visualized using nomograms, which have been trained and validated in European clinical trial populations. Data of 277 consecutive locally advanced rectal adenocarcinoma patients treated with preoperativ...
